Diamineoxidase (DAO), the enzyme mainly responsible fir the degradation of ingested histamine, is either under-expressed or inhibited. Histamine-intolerance (HIT) is, as well as lactose-intolerance, a food-incompatibility. Especially people with gastrointestinal disorders are affected. The symptoms of HIT develop slowly: Most of the affected people feel the first disorders in the early adult-age. This slowly development complicates diagnosis, because lots of the patients get used to the symptoms.

What exactly histamine-intolerance mean?
In general symptoms of HIT are caused by an overload of the organism with histamine. It does not make any difference whether the histamine is liberated from the body or ingested with food.

In healthy people the enzyme diamineoxidase degrades histamine ingested with food and thus prevents interactions of histamine with endogenous cells.

Diamine oxidase is the essential enzyme for the degradation of histamine

DAO is present in extra- and intracellular area. Especially in the mucosa histamine is degraded by DAO to the biologically inactive imidazolacetaldehyd. The activity of DAO determines the degradation rate of histamine. In case of inhibition of DAO, histamine is accumulated and provokes the symptoms of HIT

Six main reasons for increased histamine load in the body

- Consumption of histamine - rich food
- Consumption of food with high concentrations of biogenic amines (e.g. choco-late, red wine)
- Consumption or intake of histamine-liberators (e.g. x-ray contrast media)
- Inhibition of diamineoxidase
- Acute gastrointestinal infection
- Congenital enzym-damage

Determination of DAO-activity in blood
Determination of DAO activity in blood indicates, whether the organism might have problems with histamine degradation. The activity of DAO in serum or plasma correlates with the capacity of histamine-degradation of the organism. Therefore it is an adequate marker for diagnosis of histamine-intolerance and associated disease pat-tern. Using only 100 ěl plasma or serum the activity of the enzyme can be deter-mined within a few hours in a laboratory.

Therapeutical consequences in case of reduced DAO
Diet
If a reduced DAO-activity is detected, food with high concentrations of histamine and other biogenic amines has to be strictly avoided. Typical comestible goods with high histamine-concentrations are red wine, sparkling wine, hard cheese, smoked foods, sauerkraut, spinach, tomatoes.

In many cases the reasons for reduced DAO-activity are drugs, alcohol or chronic gastrointestinal diseases. Depending on the severeness of the clinical pattern, the temporarily application of H1-receptor-blockers is advised, to instantly inhibit the effects of histamine.
